Old Bus. Equipment arrival When equipment arrives at LMPP shrink-wrapped it needs to be unloaded gradually as it is being unwrapped.  This will reduce the risk of equipment tipping over.  Where possible place skids on plastic skids vs collapsed monos.  Communication to go out to employees. Old Bus. equipment left on H-dock It was found that DO drivers have been consolidating their equipment on the trucks and leaving the equipment on the dock.  Communication to be send to contractor using TCO. Sept/11 - Consolidated with Discussion - 2011.08.01 Closed    
Old Bus. Staging of monos Due to congestion in equipment revision PO-05's stacked a double stack on top of 2x collapsed monos.  The committee is requesting further investigation/information to the resolve. Sept2011 - awaiting response from Health & Safety Officer action Gale

2011.09.01 New Bus Tech Services - Safety Observation Checklist HSO Kim Gould presented on Tech Services Safety Observation Initiative.  The initiative will be rolled out across CPC.  The initiative was brought to the NJOSH in June 2011.  The Safety Observation Checklist is a tool Forman, Supt. and Managers will use to observe safe work practices on the floor.   This will be an observation of the work activity and not the employee.  CUPW has concerns with the safety observation without proper safety training (appendix DD).  The HSO Kim Gould was asked to check into DD training.  This will be included in the Matrix for the LJOSH.  follow-up Gale  
2011.09.02 New Bus Dock Leveller - Shuttle Truck interface CPC has found that the interface between the dock plates and the bumper of the RENTAL shuttle trucks does not meet Corporate specification.  The dock plate is to be on the truck 8 inches.  This rental truck measures about 3.5 inches.  A solution was proposed to the committee as part of the CTU process.  This design change would include hinged bumper plates that swing out of the way and allow the rental truck to come closer to the building and increase the interface to 8+ inches.  The committee recommended that 2 docks be replaced and that the CPC shuttle driver receive the appropriate training.  The committee also recommends a swing hinge outward as apposed to up/down. closed    
2011.09.03 New Bus IMS The committee reviewed the new IMS system in SAP.  IMS = Incident Management System.  This system is used for incident reporting  including: Near miss, lost time, property damage. closed    
2011.09.04 New Bus Prevention Campaign - incident reporting The committee agrees that employees need an information session on reporting incidents.  The committee will ask the Health and Safety Officer(s) in London to attend shift talks on all shifts to promot incident reporting. Action    
2011.09.05 New Bus Overfilling Hub Belt The committee agrees that a safety talk will be developed on the overfilling of the hub belt. Action LaGrove  
2011.09.06 New Bus Emergency Evacuation Procedures - due to weather The committee will request from the Health and Safety Officers what the proper safety procedures are for tornado warnings and other weather situations. Action White  
             
2011.09.07 New Bus Bird Wing Season Every year hunters place hunted bird parts in pre-paid envelopes.  These envelopes are placed in the mail stream.  The committee recommends that employees should be reminded of the bird wing process and that the stations will be properly stocked. Action White  
2011.09.09 New Bus C-44 shelf hieght Concerns have been raised about the height of the shelves on the C-44 racks being to high/low when using the new LFT's.  The committee will investigate if new standards have been put into place for use with LFT's. Action Ashley  
2011.09.10 New Bus Quiet Room Concerns have been raised about the cleanliness of the quiet room.  Room will be cleaned by BLJC.  Committee recommends that the quiet room be placed on the cleaning matrix if it is not already.  A sign will be posted on the fridges in the cafeteria and quiet room that they will be emptied out weekly on Fridays. 2011.09.11 New Bus Availability of cameras for investigations  Concerns about the availability of cameras for the LJHSC to use for investigations, etc.  Committee recommends that one camera be assigned to guard office to be signed out by Committee members and alternates Action Cousins  
2011.09.12 New Bus Health and Safety Orientation Concerns have been raised that new employees receiving the orientation should be given a copy of the plant rules and the emergency evacuation plan for their reference.  The committee recommends providing a copy of the LMPP plant rules and a copy of the emergency evacuation plan with the health & safety orientation. 2011.08.01 Old Bus. Signage on high dock There are various contractor issues on the high dock: 1) Drivers not reporting to the corral when their truck is being loaded/unloaded 2) Drivers removing mail from containers and stacking the empties between the dock doors 3) Safety vests and safety shoes not consistently being worn on the dock. 4) DO drivers not standing in a safe place while their trucks are being loaded.  Email sent to Dave Howlett. Sept/11 The committee recommends that the TCO performs a documented audit on the DO drivers and follows up with the contractors. Action Cousins / Ashley  
2011.08.02 Old Bus. Needlestick Procedures Concerns that sharps containers are not being replaced when needles or other items are being placed in them.  Additionally, there are concerns that supervisors are unaware of the needlestick procedures that exist.  CMS states that bottles should be removed once a sharp is placed inside.  Email sent to supervisors with CMS documentation and LJHSC safety talk.
Sept/11 - Safety talk was not delivered to all sections and will be re-sent with superintendent follow-up.  Member checking first aid boxes will now be tasked with checking to make sure that rubberized gloves are available in the area and that sharps containers have been emptied.

2011.08.03 Old Bus. First aid box at Pillar K2 First aid box at pillar K2 is being blocked on an ongoing basis.  Committee to decide on new location for box.  Location under box is to be marked.  Sept2011 - The committee recommends that the area in front of the first aid box be hatch-marked and a reminder to employees of the new hatch marks Action Gale  
2011.08.05 Old Bus. PDSL - Training There is a need to do a train-the-trainer session for the MMHE trainers so that they can do the loose load training.  Committee to investigate who may be able to provide this training.  Sept/11 - MMHE trainers are currently being trained to provide pedestrian awareness, ergo-lift, loose load, dok-lok, and new operator training.  Training will be scheduled as trainers are available Follow-up Cousins

2011.08.10 Old Bus. Truck floor incident - June 14, 2011 An incident took place on shift #3 June 14, 2011.  A BT's drive wheels fell into the floor of truck.  Committee to investigate further on incident. Sept/11 - email sent to supervisor - due to lockout, the investigation was started but never completed.  Investigation to be completed by supervisor. Action Higgin / Gale  
2011.07.01 Old Bus. Insects in LMPP Ongoing issues with bug bites in the LMPP.  Committee has requested MSDS sheets for the products used.  Tickets have been issued - CPC8356251 on July 7. Sept/11 - Still an issue in Finals - CPC9090485 opened Action Cousins  
2011.07.03 Old Bus. Fans in LMPP Fan in hub sort is non-functional.  A tech services work order has been created.  Committee is reviewing possible areas for additional fans August2011 - Repair work is ongoing.  Fan moves are on hold until floor plan changes.  At that time, the Committee will review locations for all fans in the building. Sept/11 - Plant manager was provided with a priority list of fan locations.  Tabled until floor plan changes.   Tabled    
2011.07.05 Old Bus. High level cleaning When is the next high level cleaning scheduled? BLJC advised that high level cleaning is not part of the cleaning matrix.  H&S to be contacted to determine requirement for high level cleaning. August2011 - there is no Corporate standard with respect to high level cleaning, nor is it part of the Fixed Price Agreement.  The Committee recommends that a high level cleaning be scheduled after the construction on the floor (due to PT) is completed. 2011.07.19 Old Bus. Unsafe trailers - checklist & dock irregularity book Ongoing issues with unsafe trailers being sent back to LMPP.  High dock irregularity book not being filled out regularly.  PO-5s are able to fill out the irregularity book as well as supervisors.  Irregularity book should be moved into PO-5 corral so that it is accessible at all times.  August2011 - Dawn and Barb to create book.  Sept2011 - The committee is to find/create a checklist to use to determine if a trailer is unsafe for use.  The committee recommends that management review the irregularity book process to determine why unsafe trailers continue to return to LMPP docks. 2011.04.10 Old Bus. Maintenance Shop lighting The lighting in the back maintenance shop is not adequate.  CPC will get specs on lighting and have it quoted.  May2011 - being quoted.  June2011 - ongoing. 2011.03.11 Old Bus. Dock plate coating It has been found that the coating on the dock plates of # 1, 2, 3, 4 & 10 are coming off.  A ticket number will be opened with BLJC for repair.  April2011 - ongoing.  May 2011 - ongoing.  June2011 - ticket opened  CPC8368387.  Ongoing. 2011.03.13 Old Bus. H&S training MMHE/Dock Awareness Training (1.5 day), Dangerous/Hazardous Good Training (.5 day), Emergency Evacuation Training (.5 day) and Pedestrian Awareness Training (1.0 hour).  We have new Supervisors, employees on the floor and committee members that need portions of this training.  A schedule will be set to deliver this training.  Tentative date of April 30, 2011.  April2011 - Date is set for April 30, 2011.  May2011 - Dangerous/Hazardous Goods training & Evacuation Training is complete.  MMHE/Dock awareness & Pedestrian Awareness training to be discussed.  June2011 - ongoing. July2011 - Ongoing August2011 - Once train-the-trainer sessions occur, training can be completed. Sept/11 - Consolidated with 2011.08.05 Closed    
2011.03.17 Old Bus. Truck Loading Lighting Proposal BLJC representative Steve Pfaff presented a proposal for a new lighting system for lighting up the back of the trailers on the High Dock.  BLJC proposed that CPC pilot this new system for a period of time before full installation to be completed.  Dock 9 and Dock 5 will be considered for the pilot.  April2011 - ongoing.  May2011 - Lights have been found to be too bright, need to be more of a flood light and be mounted to a breaker bar.  June2011 - awaiting options from BLJC. July2011 - Ongoing August2011 - Committee recommends a survey be given to employees to collect feedback.  Sept2011 - CPC is looking into piloting 2 of the same lights that OMPP is now using.  Feedback will be solicited from PO-5s to judge effectiveness of piloted lights action Higgin / Gale  
2011.02.06 Old Bus. First aiders Is there a need for first aiders?  It has been identified that there are an adequate number on each shift.  A list exists of interested employees, and training will be scheduled as required.  March2011 - In the summer of 2011 there will be 11 employees who's training will expire.  An info book notice will be put out seeking interest in training or re-training.  Training date TBD.  April2011 - ongoing.  May2011 - ongoing.  June2011 - Training date TBD. July2011 - Training to be scheduled in coming weeks August2011 - Training has not yet been scheduled. 2011.01.06 Old Bus. PO-5 - Parka Order PO-5s are inquiring about the status of the parka order.  It has been more than 5 years since parkas have been ordered. Feb. 23rd. 2011.01.15 Old Bus. Hole in fence - south side of building There is a hole in the fence on the south side of the compound.  Email send to BLJC to rectify. Feb. 23rd A. Chairs at sortation cases Concerns that the chairs at the sortation cases need to be shampooed or steam-cleaned.  Email sent to Steve Pfaff to confirm if this is part of the cleaning matrix. 2011.01.23 Old Bus. Ergo-lift refresher session Committee recommends that supervisors engage MMHE trainers to speak at their 5-minute meetings to speak to ergo lift safe usage / charging / etc as a refresher.  Email sent to supervisors. Feb. 23rd distribution and communication shift #2 complete.  Other shifts/sections/EE's on leave TBD. March2011 - many shifts/sections have received the re-fresher training. Shift #2 is complete.  Ongoing.  April2011 - Shift # 1 & 3 need to be completed.  May2011 - Shift #3 is now complete.  Waiting on shift #1.  June2011 - ongoing. July2011 - Ongoing August2011 - Committee checking availability of Shift #3 MMHE trainers to complete this. 2010.09.17 Old Bus. Safety vests in LMPP yard Concerns that supervisors inspecting the mail mobiles for Depot 1 are not wearing safety vests.  Email sent to Kim Gould to obtain clarification on national specifications for dock 'yard'.  Forwarded to Depot 1 for review and implementation.  Nov2010 - ongoing.  Jan2011 - OHSE opinion is the MM area is part of the yard. 2010.08.09 Old Bus. Employee lounge and meeting room - air circulation Concerns about the lack of air circulation in the quiet room and the meeting room beside the superintendent's office.  Email sent to BLJC to test the ventilation in this area. Sept/10 - Ongoing.  Oct/10 - ongoing.  Nov2010 - HSO is requesting a 3rd party perform the air quality test.  Jan2011 - ongoing.  Feb/2011 - quote has been received by CPC and is now being reviewed. 2010.08.15 Old Bus. Picnic tables Picnic tables around the plant (smoking area) need to be sanded to prevent slivers.  The table at the top of the hill by the old guard house has exposed nail heads which need to be hammered in.  Sue to follow up with BLJC Sept/10 - ongoing.  Oct/10 - ongoing.  Nov2010 - ongoing. Jan2011 - review of picnic tables needs to happen in spring.  Broken table will be removed & replaced.  Feb/2011 - ongoing.  March2011 - seats will be covered.  April2011 - Material has arrived.  May2011 - Ongoing.  June2011 - broken table was removed June 30th. 2010.07.15 Old Bus. Safety vests Concerns raised about the new safety vests - the Velcro rubs against the skin.  Employees are inquiring if they can wear other CSA-approved safety vests.  Adam to follow up with WS&H Aug/10 - The Committee recommends alternate vests and or safety t-shirts.  Adam to follow up with Kim Gould  Sept/10 - This issue has been forwarded to the National level via Ray Schindler.  Oct/10 - tabled.  May2011 - Forwarded to national via local H&S officers.  Recommendation for variety of vests/t-shirts that meet CSA standards.  Sept/11 - this issue is being addressed at a national level Closed    
2010.06.07 Old Bus. Safe lifting training Some employees have received this training, but many have not.  Most LDI resources are currently tied up in Winnipeg.  Adam to follow up to see when the training may be scheduled for. July/10 - List has been compiled of all employees in LMPP who have not yet received this training.  Training to be scheduled with LDI. Aug/10 - Names have been provided to LDI - just waiting on dates and times to schedule training  Sept/10 - ongoing.  Oct/10 - ongoing More training has been scheduled.  Nov2010 - more sections in LMPP have been trained.  There are still other areas of the plant that need to be finished (IE: PDSL and any absences - L&D to provide list).  April2011 - Ongoing.  May2011 - ongoing.  June2011 - ongoing. 2010.05.02 Old Bus. ICC Bars - Power Tailgate Vehicles Based on recent near miss incidents involving vehicles with power tailgates and no ICC bars in place, the committee is recommending that a CTU be created to recommend ICC bars be included on CPC vehicles.  This issue will be raised through the CTU process. June/10 - Andrew has met with Mike L., the CTU process has been started July/10 - National Fleet has stated that modifying trucks to add ICC bars on equipment with power tailgates will not be approved, as other options (i.e. wheel chocks) are available.  The committee is seeking further explanation. Aug/10 - ongoing  Sept/10 - ongoing.  Oct/10 - tabled. Sept/11 - ICC bars can not be added to vehicles with power lifts  Closed    
2010.05.03 Old Bus. O/S Case Insert in Depot Finals In high volume situations, O/S cases have recently been used to sort S/L mail in Depot Finals.  A proposal from Depot Finals section employees for an insert into the O/S case to better accommodate S/L mail was presented to the committee.  A prototype has been developed.  A CTU has been created to evaluate the prototype.  The committee recommended rounding off the corners of the prototype to reduce the potential for an injury.  Aside from that issue, the committee is in agreement with the potential to use the device and awaits CTU approval. July/10 - Ongoing Aug/10 - ongoing.  Mike L awaiting response from WS&H  Sept/10 - Ongoing.  Nov2010 - CTU is awaiting final approval.  Prototype is complete and ready for production.  Final version will be presented to H&S committee when completed and then implementation.  Feb/2011 - This CTU has been placed on hold at national. Sept/11 - This item is cost prohibitive and will be closed Closed    
2010.05.11 Old Bus. LMPP MMHE Draft Policy A new draft Policy was presented to the committee.  The Co-Chairs will review and will sign-off prior to posting and distribution to the high dock employees. July/10 - Ongoing Aug/10 - ongoing  Sept/10 - ongoing.  Oct/10 - ongoing.  Nov2010 - ongoing.  Feb/2011 - ongoing.  March2011 - ongoing.  April2011 - Ongoing.  May2011 - A sub-committee has been established and will meet in mid June.  June2011 - Review of the policy has taken place, but not all stakeholders were in attendance.  Further meeting to be scheduled in July.  Committee to review draft. 2010.03.01 Old Bus JHA - MMHE Assignments The JHA has been completed for BT#3 on Shift #1.  A schedule needs to be developed to complete the JHA analysis process for the remaining PO5 work elements. Nov - ongoing.  Jan - there seems to be increased fowl traffic on the PDSL dock.  Committee requests to have BLJC inspect the bird deterent unit for functionality.  Mar./10 - BLJC is reviewing other potential options to deter the birds from nesting in the area.  Issues exist above the high dock pedestrian door and in a hole above dock #8.  Apr./10 - Paperwork is being submitted to BLJC for additional work to reduce the bird issue on the dock.  May/10 - The area has been power washed.  Permanent solution is being researched. June/10 - Ongoing.  July/10 - Ongoing. Aug/10 - Ongoing Sept/10 - Ongoing.  Oct/10 - tabled.  Nov2010 - committee is requesting an update as spring is coming and this issue will araise soon with nesting birds.  Feb/2011 - issue is starting up again as spring is approaching. 2011.09.02 MLOCR PHSR - October 19th Mike Leutheusser gave some information about the Pre-start Health and Safety Review (PHSR).  On the morning of October 19th, an engineer from Stantec (engineering firm) will be doing a PHSR and a risk assessment.  PHSR ensures that the specifications of the new machines meet the specifications set out by the Corporation and is compliant with all health and safety requirements.
